As indicated in the Security Clearance Instruction letter you have already received, there are additional forms that must be completed. These documents are the Security Acknowledgement and Fair Credit Reporting forms. These documents must be submitted in hardcopy format to the address provided in the Security Clearance Instruction letter. Links to these documents are provided below. As also indicated in the Security Clearance Instruction letter, it is possible that you also may need one or all of the remaining four forms below based on the explanation provided with each. These forms must be submitted in hardcopy format to the address provided in the Security Clearance Instruction letter. If you need to access one of the forms, click on the appropriate title.

• DOE F 5631.34 (Data Report on Spouse/Cohabitant) -- This must be submitted if you are married or live with someone in a spouse like relationship.
• Gov't of Australia -- This must be submitted if you resided or worked in Australia for 90 days or longer. This is a release form that Australia requires before releasing any information about your time in their country.
• Gov't of Canada -- This must be submitted if you resided or worked in Canada for 90 days or longer. This is a release form that Canada requires before releasing any information about your time in their country.
• Resided or worked outside the US -- This must be submitted if you resided or worked outside the US for 90 days or longer. This is for informational purposes relating to your background investigation.
